For me it was Mount and Blade: Warband. I bought it on a Steam sale hearing a few good things and within a week I had probably a good 50 hours in it. I've never played a game that hard.
Spelunky had me going for several months, and Binding of Isaac is my most played game on Steam so I'll say those 2. There was a good month long period where I played nothing but R-Type Final as well.
X-COM during my college days. I was obsessed with that game to an unhealthy level.
Luckily I'm now able to control it and can play a reasonable amount of time. God knows what will happen October 9th when the reboot comes out.

Grand Theft Auto III, back in April-August of '02. I failed the final quarter of my freshman year of high school because of it, but I didn't give a shit. I didn't have a sound system, so I used my Dreamcast to play CDs through the TV as I was playing it. I made many a custom playlist for it too. Sometimes, I would just jump the same ramp over and over, for hours. Or, I'd spawn a tank and drive around the baseball stadium, mowing down every living thing I could. I loved it. I amassed something like 742 in game days before I finally stopped playing. I definitely got burnt out, but I can easily say it's the best GTA ever. They just don't have the staying power.
Spelunky and The Binding of Isaac are the big ones for me. I just can't stop playing them... The randomization of the levels is probably a key factor in their addictiveness, since it's like a new game every time. I keep wondering what I'll come across in a new game and just keep playing and playing and playing.
The most play time I ever put in a game was Pokemon Diamond, and I invested about 400 hours. But the most addictive game I've ever played is probably Jef Set Radio Future. It is my all-time favorite game. I love that game so hard.

I'm a sucker for loot based RPGs. Borderlands and Champions of Norath were the two big ones for me. Beyond that, the typical WRPG stuff (Fallout, Elder Scrolls, etc.)

I was addicted to dragon Age: Origins, it's still my highest played game with over 500 hours on steam.
Ashamedly, Tiny tower. As for a game in which you actually do things; Bad company 2, spent over 200 hours on that. The most I've played any other game would probably be 60 hours on GTA4
X-Com in a big way. Loads of hours of my life stolen by that game. Still to this day. Oh, and Uncharted Waters 2 for the SNES. Love that game, again, still today.
Tekken Tag and Tekken 3 was some of the few games I had a total blast fighting pros, button mashers and even non fighting game players I knew.
Easy. The Elder Scrolls III: Morrowind.
I distinctly remember sitting getting back to my dorm from class around 4:30pm with some Burger King for dinner. The next thing I remember is being absolutely baffled as to why my alarm clock was going off. It was 10:00am the next day and time for class.
The best part is my roommate had stayed up the entire time watching me play as well.

My first Zelda Game Oracle of Seasons, dunno how many hours I put into it, but I played it non-stop and obsessively, then I played the other Game Boy games (including the remakes of the SNES). Stopped at Minish Cap, and never played any more Zelda game.

Morrowind. I threw on god mode and spent the whole time killing every single person I found and stealing all the white pillows in the world. Bethesda RPGs turn me into a hoarder. It was teddy bears in Fallout 3.

I was obsessed with this game after I beat it. I spent my middle school years reading the old Odyssey of Hyrule page every day, reading rumors and secrets and glitches, playing the game with Gameshark codes doing all kinds of crazy shit. Remember all those silly rumors about how the Triforce was an obtainable item in the game? Yeah, I ate up all of that. I know the game like the back of my hand because I made that my mission.
2nd place probably goes to THPS2.

Fallout 3 and New Vegas 6 to 7 180 plus hour play through in each game, and still I love going back and pouring more hours into those games.
Let's start at the beginning (for me): SMB3, Contra, FF, SF2, MK2, Phantasy Star 2 n 3, D&D: EotB, Gunstar Heroes, Doom 2, Oregon Trail, Xcom, Fallout 1 & 2 + Tactics, D2 (for like 6 yrs), Tekken 3, Action Quake 2, CS, Shogo mp, Red Faction 2 mp, Timesplitters 2 mp, Battle Chess, Battlefront 2. Tony Hawk. All of em to thug2.
Then I took a break from gaming and got addicted to:
Fallout: New Vegas, SMT series, MW2 and Blops, Spelunky (original), Action Fist, UFO, Tekken 5, SF4, BoI and Unepic. Sorry to any I left out.

The Binding of Isaac, hands down. Never thought a $5 game could entertain me for well over a hundred hours. I'm completely and hopelessly addicted with no signs of slowing down.
Super Meat Boy (XBLA) and Spelunky (XBLA) currently come in a close second and third.
Years ago it was definitely Final Fantasy Tactics Advance (GBA). Put over 300 hours into that before getting burnt out.

I just beat challenge 10, "The Purist" by the skin of my teeth. I haven't felt this proud of my gaming skillzz since I got Golden God in Super Meat Boy.

By FAR, Rainbow Six Vegas. Getting together with 3 friends and playing countless rounds of Terrorist Hunt was the most addictive fun I've ever had with a game in over 25 years. We played Terrorist Hunt so much and for so long that we were able to beat any level in under 3 minutes on Hardcore with just the Mac 11.
Anyone who knows what all that means knows it's damn impressive.

Based on number of hours spent, Guild Wars. Based on time spent playing despite that fact that it sucks and I dislike it, because I desperately wanted it to be a better game (and because apparently I'm a masochist), SWTOR.
