The increasingly sedentary experience they provide is my largest problem with online multiplayer right now. The best online gaming experiences I've ever had were while playing Timesplitters: Future Perfect on the PS2.
No matchmaking meant that you could lobby hop, which lead to extended communities of people who were familiar with each other without having to be on everyone's friends list.
Cross team communication meant that you could have truly social matches, fostering a tight community.

The lack of some kind of industry standard system for matching people online with similar interests and maturity levels. When teamwork functions well, it adds an incredible theretofore missing element to the multiplayer experience. It would be great if the people who actually cared about communicating well and helped to foster synergy among teammates could be recognized and rewarded. Maybe a fish-ladder type of social effectiveness or teamwork effectiveness rating system could be constructed that would eventually match you up with people who like working together and are good at it, if that's what you want.

I'm going to have to go with the teamwork thing. Even in a game like TF2, playing without friends, the match can be fruitless when people don't work together.
TF2 is actually one of the better games because individual achievement isn't so important like in CoD where apparently according to most your KDR proves how good you are.
There needs to be a way, aside from chatting that will get those people who don't own a mic or don't use it to actually work together and they need to be rewarded for it somehow when they do.

Kids playing games that aren't old enough to play said game. I think there is an age limit not because of the content but because we shouldn't have to hear them.
Reliance on other people for any teamwork oriented team-based games or most especially co-op of any form.

Consistant spawn points. I think if the starting spawns were random than the bottleneck and camping elements would be cut down; you wouldn't know which direction to run / look / toss grenade at the beginning of every match.

Nothing is more irritating than hearing the squeaky voice of a preteen or child being loud and obnoxious, spouting racist and homophobic slurs because they just learned them. And it's painful to know that there are so many parents out there who would be irresponsible enough to just toss their kids on XBL or PSN with proper (and improper) adults, especially considering it's almost always M-rated games.

I'm not saying everyone who plays online does it, but there are a lot of people that really don't care for single player campaigns and instead rarely branch out from their safety zone of online multiplayer (online shooters especially), meaning a lot of creative and different games go overlooked by a lot of people just because they lack online multiplayer

The fact that single-player campaigns are more of an afterthought because of OM's popularity. But maybe with Skyrim's runaway success there's change on the wind...

I'm sorry but there should be no wait time between matches. There is literally no need for it. Jump in jump out(maybe it's just my PC history) you want to change your load out jump out of the server for a second and do so.

The fact that no matter how much people complain about camping, not much is done to force a more dynamic game where it is not possible.

Starting CTF in Halo on the latest massive Blood Gulch-clone map, the other team captures one flag, and my whole team quits, leaving me alone against the entire other team on a huge map. And they grief me mercilessly for half an hour.
Basically, anytime my whole team quits, one after another, and I'm the last man standing against a huge team. That is my annoying thing.

But there are so many to choose from. I guess I'll go with lag as my biggest problem. Mostly because my internet connection at my apartment is utter shit.
The people. Mainly the ones that insult you whether they win or lose and use a biggot's mindset. It's a constant reminder why we can be glad 2012 is the last year we'll have to deal with them.
The fact that playing team-based games (think BF3) always results in having to play with people that NEVER use microphones whereas less team-based experiences (CoD) always results in morons screaming in your ear or singing the whole fucking time.
Tacked on multiplayer. I hate it when I play a great campaign, with a great story, graphics, physics, and game play only to find out a game that could've had or should've had a decent multiplayer was thrown together at the last second. Example: Far Cry 2, Ubi seemed to have focused most their effort on singleplayer but had the online map editor which is great for multiplayer but when came down to actually playing the multiplayer it was crap! The equal opposite can be said about some multiplayer games, example the recent Battlefield games, fun online -tacked on singleplayer.
I guess the one thing that I find pretty annoying is when people take it far too seriously. I play a match of Starcraft 2 or Halo online, and the thing that draws me away from it is when people play just to win. I know, playing a match of Starcraft to win is something that everyone does, but at the same time I think that it's a little much when they just do a cheesing tactic in order to win. It's cheap, unfair, and it takes the fun out of the game for everyone.
