There are undoubtedly weirder and more ambitious mods for Fallout: New Vegas, but this one -- put forth by the game's lead designer Joshua Sawyer -- is interesting in its specificity.
He calls it "a general difficulty tweak and bug fix mod," which provides a "more challenging" New Vegas and fixes "a bunch of the more obvious balance and .esm incompatibility issues." You can read through the lengthy list of individual tweaks at the above link.
A word of warning: you'll need all of the downloadable content for the game, and Sawyer says you may run into issues with other mods installed. Still, seems like a worthy endeavor if it's a harsher Mojave Desert you're craving.

This sounds interesting. I've been wanting to play though NV again.

but seriously, from what ive heard from friends, and from a video on youtube that shows you like 30 ways to kill a hooker in dead vegas with 30 different, psychotic guns, i'll be happy for a difficulty bump.

Only a moron would waste all that time and effort to make a game harder. If you really wanted a challenge you should just play blindfolded like I do. You all are a bunch of idiots that probably couldn't even turn on a light switch without directions!

No it really shouldn't. Effective use of a cover system would then mean that the entire wourld needs to be covered in chest high walls.
Not to mention something like fallout where cover doesn't make a lot of sense when you have non human enemies which rush like deathclaws
Robotic and mutant enemies which by sheer size or buildtype aren't goin to be able to use it realistically
Effective cover where possible to use against enemies with ranged weapons like the buildings cars boulders or others I know there are melee enemies but using cover would be actually beneficial to survival and in a survival scenario shooter like fallout it just seems like its missing
