I play games for their escapism generally. Realism doesn't really factor into it for me.
The rapid advance of graphics technology was always going to bring this issue.
I do believe that there should be no boundaries though (just as with films and book), and its possible we could say that a lot of boundaries of gore and viscera have already been crossed.
Its quite hard to shock gamers now, as we've seen most of it. Developers will have to come up with new ways, to be better than the movie shock likes tactic.
Gears of War, Dead Rising have done their bit for the gore and violence side which are very visual. Sex or any kind of relationship intimacy has yet to be done, let alone to a convincing standard, but God of War and Mass Effect, and more recently GTA4 have at least attempted to cover the subject as part of a game. Not bad so far, but a long way to go.
Realism will only become more relevant as things progress, and we are at a good level right now, and when it reachs its summit, things might change and split off to differnt art styles, eg: No More Heroes, Jet Set Radio and so on.. The mainstream non gamers will obviously notice games more as they reach that kind of graphic detail, hence the uproar of Gears 2 chainsaw and meat shield play mechanics.

thank you for your well written comment, and i see that i was merely overstating this subject and do not have to worry about it.
They'll never be able to give characters souls, so the more real they get, the freakier it'll get (see Uncanny Valley).
In my opinion, games should be progressing in fun, mechanics, interactivity, story, art style, physics, animation, and innovation. We've got a world to live in, so let's make games as different and diverse as we can.
I'm really not interested in shooting someone and watching their guts realistically drop out and dangle on the floor as they frantically try to stuff them back in. I don't think the level of possible violence in video games needs to go any higher.
@eschatos i agree with you, but don't you think that even pulling the trigger can get that difficult?
and for the sake of our society i hope that level of violence will never be reached.
