I'm not really sure what people expect from sequels. In terms of AC Revelations - I enjoyed the game. If finished the story well and the gameplay was only changed a bit from AC2 so the game wasn't a complete change up. It was what I hoped a sequel would be.
Modern Warfare 3 introduced support streaks. This was a substantial enough change for me to actually play the online portion of the game and enjoy it for a change. Frankly I like the annual Call of Duty games. The give a good single player campaign and then add in the expected multiplayer with a few change ups to make it interesting, but not enough to completely change the game - what a sequel should do. The Modern Warfare series is still one of the best series out there... the games are very playable, enjoyable and they work - something that few shooter games can claim (so many are glitchy pieces of crap in the online with very poor added on campaigns - Call of Duty is one of the few series worth buying for both the campaign AND the multiplayer).
Fable 3... same thing. It certainly wasn't the best game in the series, but it was still enjoyable and stuck more or less to the formula of the first games.
In any series of games there will be some games better than others. The thing is that the expectations are that there will be some minor changes to make them new... but enough there to keep them comfortable. I think when you compare these series to something like the Resident Evil series it's easy to see the difference in how the series were handled... RE tried to do something completely different - and it didn't work out so well. :(
.. being "safe" isn't always a bad thing.

I should mention that I love all Call Of Duty Games until MW3 . They seemed to have balance and variety , while MW3 felt too much . Even Black Ops had some calmer moments , while mw3 couldn't calm the fuck down , OH OH , LOOK BIG 'XPLOSIONS , HEY THE DEATH SEQUENCE IN COD4 WAS A REALLY STRONG MOMENT , HERE'S 2 CHARACTERS DYING THE SAME WAY , ITS WAY MORE EMOTIONAL COZ IT HAPPENS 3 TIMES , RIGHT ? Also , the topic is "Good Games , Bad Sequels " and I would never dare say that mw3 is bad , its just really dull and soulless compared to the older games in the series . All and all , if I was a reviewer , I'd give it a solid 5/10 .
one more thing - I'm all for judging games for what they are , thats why I don't hate games lie Silent Hill Downpour , but when the game clearly wants to be it's predecessor , I can't help but respect that and compare it to the old ones .
Sequels are hard to do it has to be similar enough for it to have the same atmosphere and personality of the previous games, but it has to be different enough to stand on it's own legs. There's been many games that I played that were well made good games, but compared to their predecessors I was disappointed. Some examples that pop into my mind are Uncharted 3, Portal 2, and Metroid Prime 3.
