They changed the picture way before Meet the Scout. They only used that first one for Meet the Heavy.
The 2nd one looks much better.

The first picture is everything from the Beta version. The second picture is the finished version. If you have it for the PC, listen to the developer commentary..they talk about some of the aesthetic changes they made.
Yes, quite old. I do wish they had the giant syringe as a medic weapon though....
They're updating the picture to meet the actual look. The first one was just concept art.
