So, in case you didn't know, Valve has announced a new "Realism" mode for Left 4 Dead 2. They have, of course, told us nothing about it, so that leaves us a few weeks to speculate until they release the details. The only things we really have to go by is that it's going to be realistic, and it's for Portal speedrunners.
Now that I have some time to think about Realism mode, I'm going to list some of the ideas I've come up with, the things I would consider putting this mysterious mode.
Look at Expert mode as the foundation upon which I will build. For example, 4 hits from special infected incap you, hunters almost instantly incap you, etc.
1. Health packs do not heal zombie related wounds. Health instead regenerates very (I mean VERY) slowly, capping at 15 points. Why? The zombies punch. Putting a bandage on after getting punched doesn't make you feel better. You'll have to wait to feel better.
2. Gun recoil.
3. Not all shots will 1 hit incap teammates. A shot to the head kills, stomach will incap. A shot to the arm will dramatically increase gun recoil. A shot to the leg will dramatically slow the player down. A lot of health will be lost regardless, and the player will stop regenerating health and start bleeding out slowly. Only a healthpack can stop the bleeding and return the player to normal. Oh, and 2 shots kill unconditionally.
4. There will only be 1 healthpack per map, which can only be used for treating puncture wounds.
5. Pain pills have a side effect, such as blurred vision, and dizziness upon wearing off. They cannot be taken too often, as you can overdose on them.
6. Headshots 1 hit kill everything except witches and tanks.
7. Headshots do extra damage to tanks.
8. Setting a tank on fire does direct damage to the tank (enough to kill it in 1 minute), rather than putting it on a 40 second death timer that in unaffected by damage done. The tradeoff to the tank being easier to kill is...
9. One hit from a tank kills. Nobody could survive a swipe like that.
10. No crosshairs. Iron/laser sights only.
11. There is less ammo, and it is measured in clips rather than total bullets.
12. Guns jam realistically based on their real world counterparts (As in AK47 never jams, and M16 constantly jams).
13. Melee fatigue increases in an exponential amount with the amount of zombies being hit. time between melees determined by number of zombies being meleed squared after 5 attacks.
14. Explosives send out shrapnel about 20 feet (stopped by obstacles including zombies), which can cause puncture wounds that require a healthpack to treat.
and I suppose I'll stop my speculation at
15. Boomer bile, regardless of if it gets on a survivor should call the horde. The horde will just go wherever the bile is.
So, that's what I've got so far. Any other ideas out thar?

I mean, if you were to actually encounter a zombie in real life, would you really be able to hold your own? I mean maybe you could flay at it with some sort of compromised implement but you still wouldn't stand much of a realistic chance.
Also I think all cars should have alarms, no closet re-birth and I don't really know how to describe the last one..the character models should be solid so you can't run through them like ghosts (survivors sitting in corner)
