Beating my friend on CoD2 multiplayer on the 360 whos awesome at the game with a PAD . im a PC gamer who cant stand FPS games with PAD controls so it was a big deal for me. He has beaten me every single time before and since tho, lol.
Beating the Supreme Commander SP campaign even though i am not great at RTS games generally. The final levels are hard because you spend loads of resources building up a defence from attacks from a certain direction, then it opens up the map showing a counter attack coming from a completely different direction.
Beating Braid without using a FaQ or anything.
Im sure there are loads more but i cant remember. Also beating older (8-bit/16-bit) games is probably more of an accomplishment than beating more modern games.

sephiroth is easy in kingdom hearts 2, the best thing to do in that game is to finish the arena completely
Fallout 3 - The Nuka Cola Challenge. 'Nuff said. Also finishing the career on the original Rock Band on Expert.
One of mine I think is getting all of the Legendary Pokemon and finding the flipping things was a great achievement, as they are 1) Hard to find and 2) Hard to beat.
Completing Word at War (even though I'm not a fan of ti anymore) on Veteran as well as shooting the general with a pistol instead of a sniper rifle.
Beating Super Mario World and Yoshi's Island on the SNES.
Completing the Paris level on Worms Armageddon on the N64 (easily the hardest challenge ever invented), you can't mess up a single go, the AI is on hardest and its 1 worm versus about 5 or 6.
Finishing the insane world that is Discworld 2 and defeating the dragon.
Finishing my 24 hour race on Gran Turismo over the course of a few days of non stop playing.
Defeating The End on Metal Gear Solid 3 as well as getting his amazing Moss camouflage.
I only have one real gaming accomplishment that I can recall off the top of my head. Beating Contra on the NES without the Konami Code or the Game Genie. Did that once when I was about 13. Have never been able to do it since. :P
Completed Wind waker's figurine library. I haven't met anyone who did this beside me.
Completed every chapter on very hard in RE : Outbreak File 2. Desperate times is soooooo hard. Crazy. Damn gas.
Completed Metroid Prime on hard. Omega pirate is harder than the metroid prime itself.
My brother and I usually challenge each other to speedruns of new Zelda games when they come out. I obliterated him at Majoras Mask... which ruled, because he kicked my ass at OoT before it.

My apologies, I don't frequent the forums so I didn't realise that this had already been brought up there.

I guess one that stands out to me was based on sheer luck. At the end of FF7, Cloud had just leveled up off the last Jenova boss. The next thing I know that his HP has hit 7777 and he goes crazy all over Sephiroth, making it way too easy than it already was.
Going back and finally completing Mega Man 9 was big for me. I initially bitched out due to of the difficulty and set it aside for months out of shame. It took me a while to really get back into that retro mindset, and to re-learn how to play a brand new Mega Man game. I have since redeemed myself.
I was also super happy when I got my final achievement in Dynasty Warriors: Gundam, where you have to kill 100,000 total enemies. I had to work on that for a while, and I'm at about 76,000 on DW: Gundam 2.

finishing ff12 with all missions beat, 47 trophies and characters all at lv96 before i gave up playing it.
ffx-2 100% that shit was really hard.
mgs3 no kills and no alerts on hard, it took me a month of saving/reloading and crawling almost the whole game to do it. you try to take out The End with just a tranq gun and see how long it takes to "get lucky".
