No matter how good it is, someone will either find or imagine a reason to be disappointed. That having been said, I'm going to feel a little let down myself if nothing I do influences the core narrative.
They could learn quite a bit from Alpha Protocol in this regard. Massively underrated as a RPG, imo. The decisions in it change nearly everything in the story, including the bosses you fight, your allies and enemies, and the ending.

The unfortunate part is that crafting an experience like you're talking about is more or less inherently at odds with the company's financial goals. If they make a great game with or without these things, their sales are unlikely to differ by much, but the cost for doing it the way you suggest could be much, much higher.
And if it's one thing I've noticed with games, it's that if something exists in the game, it's there because the developers hope a lot of people will see it.
That being said, if they do pull it off, it could be a landmark achievement for gaming. The Rachni were fine, but I want to see more done with the genophage.

I am really, really hopeful that your decisions in two will have meaningful consequences for the third installment. I am in love with the idea of a narrative carrying over through three games. I think with subsequent games, Bioware will get better at including these sort of things as part of the overall gameplay experience.

The cameos and emails from NPCs from the first game were nice touches, but many of my decisions seemed pretty inconsequential.
I'm looking forward to the (hopefully) serious repercussions in the third game, though. Guarded optimism seems to be the way forward, as you say!
