Yes, it is exactly as it sounds. No tricks to it, no bells or wishes. Heck, it doesn't cause any damage.
The main purpose of this item is to
hinder. It blocks your position like any other smoke item.
The only "special" feature is that can be picked up and thrown again multiple times, even as the smoke has started.
So how do you like them apples?

And the item, well, I can see it now. I'm trying to beat classic mode on hard. I'm fighting against Captain Falcon or some other character that always manages to kick my ass. My damage is up to %150. His is up to %125. I get a crate. I break the crate.
Three smoke balls. I throw them all in desperation that they will do something, but all I've done is give Captain Falcon some cover so he can charge up Falcon Punch in secret.
And of course, I get killed.
Just like the invisibility item, this item will likely be meaningless in single player, which is all I ever play.
For those multiplayer kids, I'm sure it will be a good time.

Hey there could be a hide behind the smoke bomb solid snake stealth level in single player...then again there could also not be.

None of my friends like fighting games. In fact, they hate them. I did get some of my friends to play Pac-Man Vs with me, but that's about it for me and multi-player.
